how would you take an apical pulse? i've heard this is the most accurate, but never looked into it, as my doc recommends i take my pulse once a day.

Take it at wrist: The apical pulse is taken at the apex of the heart. This is taken by using your stethoscope. The wrist pulse is just fine and is free. You do not need to buy a stethoscope. This will do just fine.

Depends: Normal depends on age and activity. At rest, 50-100. With peak exertion, the normal peak heart rate will be in the vicinity of (220 - age in years).  Example: if you're 41, your normal peak hr would be around 179.  This is just a rule of thumb. Many people can exceed this with strenuous exertion.

BP122/68 pulse 95: BP is fine pulse is a little high could be dehydration ,heat ,exercise ,stress, anxiety , pain.  Would hydrate ,relax and repeat. Note  pulse and BP change second  to second so would not worry  some people have baseline higher pulse , occasionally high pulse is due to  thyroid issues, very ,very rarely clot in lung if associated breathless, and chest pain  F/u with doc check thyroid d-dimer

No: normal pr is 60 - 100. but if ur PR is high lying in bed it must have a reason e.g.: anxiety, dehydration, anemia or less likely CHF. So Check with a cardiologist.

No, but explainable: Hr is influenced by many factors (level of sympathetic nerve (adrenalin) activation, state of body physiologic stress, thyroid levels, hydration status,  and cardiopulmonary conditioning.). Is it regular or irregular?  Irreg abnl heart rhythms (like atrial fibrillation which produces an irregularly irregular rhythm can occur.  An ecg/exam can define your rhythm and a doc eval shd provide a cause.
