stable foci of nodular pleural thickening measuring 3 mm along right minor fissure  and 4 mm along right lateral inferior?

There are lots: of questions here. While you don't say, this sounds like a report from an imaging test such as a CT/MRI of the lung. The pleura is a thin membrane that covers the lungs. Thickening results from scarring, infection, tumors, etc. Unfortunately, most NODULAR pleural thickening is the result of cancer..either metastatic from the lung, breast or elsewhere, or mesothelioma. Your doctor can elaborate.

: That is an abnormal CT finding but the interpretation depends on the medical history, physical exam and other test results.

Observe: a with a CT scsn of the chest in 6  months to look for progression. Isolated pulmonry nodules are quite often benign.

Chronic changes: The description is consistent with chronic changes, probably secondary to infection. With time the findings may improve. If you have sinus symptoms, consider an ENT appointment and nasal washing culture.

Otolaryngologist: In view of these findings it appears that you have had repeated infections of your middle ears and repeated nasophayngitis through the years with build up of reactive lymphoid tissue and cyst formation at the back of the nose. An Otolaryngologist is best to advise and treat  you and follow you for any ongoing issues involving your nasal passages and ears.

10 days post sphincterectomy. i've developed two small fissures on outer most part of anal tissue. is it possible to acquire new ones? will it heal?

can anal fissure lead to scar tissue that makes it difficult to pass stool? had issues with fissures on/off for a few months. now for last 6mos extremely difficult to pass stool. can only do while standing/hunched. if i strain i get hemroids easily?

: A stool softener may help. You would not have this much difficulty in passing soft stool, even if there was scarring.
You may take a fiber laxative like Metamucil and drink enough water daily so that your urine is colorless. For the long term you may try to increase your intake of fiber foods. Consult this site for fiber foods.

Lymphomas/Leukemias: Lymphomas like Hodgkin's/Non Hodgkin's Lymphoma OR some Leukemia of lymphoid cell lines like CLL(chr lymphocytic leukemia] , ALL/acute lymphoblastic leukemia etc

Yes: The spleen is an organ structurally like a large lymph node. It acts a filter for the blood circulation.
