PRWP: PRWP or poor R wave progression can be a sign of a few things: most common problem I see is artifactual: simply a matter of suboptimal electrode placement of the EKG either due to sloppy technique or else breast tissue (in women). PRWP can also be a sign of a previous anterior wall MI or heart attack. Lastly, some conduction abnormalities such as left anterior fascicular block can cause this

Holter: A holter monitor is primarily used to look for abnormal heart rhythyms, not so much to look for ischemia. Each ECG lead looks from a different angle a the heart while the Holter from only one angle.

i went to the cardiologist bc of an abnormal ekg saying inferior ischemic. the cardiologist ekg was normal but he wanted echo and stress test to make sure.  it been 3 days. if he saw something wrong like heart failure would he let me know right away?

Ischemic finger: I would. If you truly had an "ischemic" finger (ie., it almost died from lack of blood flow), i would want to be checked for certain rheumatologic conditions that can cause vasculitis (scleroderma, wegener's, polyarteritis nodosa, lupus, etc.) and for cryoglobulinemia (blood clots in the cold). It may just be "raynaud's disease", but it is severe; treatment with a calcium channel blocker may help.

what if there are ekg changes during surgery? what does that mean? i had a knee replacement surgery and my ekg had changes during, i am 59 and female.

Needs Follow Up: It means that you  when your body was under the stress of anesthesia, your heart didn't tolerate as well as the next person. You should follow up with your primary doctor or cardiologist. Likely they will do some more testing like a stress echo.
