Is one of the: Common problem  with that medication  , try a smaller dose  with coffee  , if it is not helping  you  , seek alternative therapy ( including surgery )  through your doctor

NTG Headache: If you have no contraindication, take acetaminophen.  Try 650 mg first dose.  See how it works.  Can take 1000 mg as a dose.  Do not exceed 4000 mg per 24 hours

Surgery: Nitro has about 70% success , but to excise that annoying tissue and reach almost 100% success rate do a lateral sphincterotomy with pile excision.

Dilatation of vessel: All nitroglycerin forms are vasodilators of vessels of the body, including the brain. When there is vaso dilatation of brain vessels, headaches happen. Depending of NTG forms they may last for minutes to hours and they are uncomfortable but not dangerous

Nitrates will all: cause this vasoreactive response. Perhaps your health care provider could adjust your maintenance therapy (beta and calcium blockers) so that you won't have angina and thereby not need NTG.
