Not necessarily.: For uncomplicated bacterial uti, if both organisms are susceptible to the antibiotics, there is minimal difference between the two agents. Both agents are generally good choices for initial therapy. However, if there is a risk that the UTI has progressed to become a kidney infection, Bactrim (sulfamethoxazole and trimethoprim) is superior, because Macrodantin does not penetrate the tissues of the kidney well.

Urine Culture?: I imagine by now your doctor has done a urine culture.  That should show exactly what the bacteria is causing the infections, and what antibiotics it is sensitive to.

i was diagnosed with a uti ... no symptoms. i was prescribed nitrofurantoin and bactrim (sulfamethoxazole and trimethoprim). both bad side effects. can i do something without taking meds?

Call your doctor: The most common side effect of these drugs is allergic skin rash, especially with Bactrim; I would be suspicious that any other symptoms are not actual drug side effects. But even 2-3 days treatment sometimes is enough for UTI. If you need additional Rx and truly cannot tolerate either of these drugs, several other antibiotics would work. Contact your doctor and follow his or her advice about it.

this is regarding my partner (23/f). she has been treating uti symptoms for 3 weeks with no change. she had 4 urine samples all came back negative. tried trimethoprim for a week and nitrofurantoin for 3 days. can't sleep from the pain?

Take your meds: as prescribed,  rest and push more fluids, keep the follow up with your doctor, if not better in few days go to the hospital, you may need a urologist input, at times UTI takes more time to get over, wish you well

Your question: Your mild pelvic and back pain may still be due to UTI since it is only day 2 of treatment. If you still do not feel well after 5 days contact your doctor and arrange for ultrasound or or other imaging of the kidneys to make sure you do not have a stone or other obstruction.
