Try new medicine: There is a new prescription medicine available that is supposed to be effective for nail fungus. However it has to be applied to the nail for at least two months or longer. Please see your doctor for a prescription.

You : You likely are dealing with an infection. You need a medical evaluation right away. You will likely be given a course of antibiotics and possibly have the wound incised, cleaned, and drained. In the meantime you can take advil (ibuprofen) or Ibuprofen for the pain and inflammation.

: She will believe what she wants. You may just be an easy person to vent on so consider it a side effect of the relationship. The facts are that you can acquire fungal spores from people/pets/dirt/other environment. They grow so insidiously it is hard to pin down where they came from.

i have bad itching on the tops of my big toes in the evenings but skin looks fine. i've been taking terbinafine for two weeks for fungal nail infection (in both big toes) and also using loceryl. it's absolutely maddening. is it to do with the medication?

Probably not: The itching probably has more to do with the fungal infection.  I would take 25 - 50 mg of benedryl at night to help the itching.  Cool compresses with Domeboro solution may help as well.  Benedryl cream is another options.

See a dermatologist: First be sure of the diagnosis. Terbinafine pill taken 1 a day for 3 months works most of the time. Prescription costs $10. When it works, there is a better nail in a year. Laser costs $1200, not covered by ins., better nail in a year if it works. Topicals are much less effective and much more expensive. Meanwhile ventillate your feet, keep nails short, take sox off in bed.

Toenail removal: However the nail fungus (which is what I'm assuming we're dealing with here) may return when the nail does.  The drops can take 3-6 months because the nail grows slowly and the nail already damaged will not improve, it has to grow out and be replaced by normal nail.
