i have a bump on my right thumb joint.  sometimes i feel a sharp pain, mostly it feels numb.  what can this be?

Likely ganglion cyst: These bumps are often from the joint forming a bubble of fluid.  If it presses on a nerve it can cause numbness.  If it is getting worse, go see a hand surgeon

Sounds like: carpal tunnel  but other nerve issues can cause this  go to your doctor for an evaluation

how long one is supposed to take 40mg febuxostat to keep uric acid levels below 6?? mine usually increases again after 1.5 months, i dont suffer from gout but i have stiffness in my right foot joints accompanied with burning sensation in thumb joint?

: If you have to use febuxostat for controlling high uric acid level you have to continue for the rest of your life unless you are able to keep uric acid intake to a minimum which is not easy. The medication probably has full effect after 2-3 weeks of taking it. If uric acid is not lower than 6 after 2-3 weeks, dose needs to be doubled.

i have 2 dermofibromas, 1 on top of foot, 2 an inch above thumb joint both where skin is thin. mine do not dimple when squeezed, cos skin is thin??

Wish you good health! - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

An ip Jtlw fusion: at the thumb tip or an MP joint fusion at the large knuckle typically cannot be taken down and made to move. However if you have had a fusion at the base of the thumb or what is called the CMC joint then at times that fusion and be removed and the  thumb stabilized with some type of tendon transfer or inter-position and motion can be restored partially. There are many variables to consider first.
