Ac joint: Probably coming from the ac joint. This is the joint between the collar bone and the acromion. Its common to develop pain and popping in that joint with push-ups. If painful see a doctor for evaluation.

Shoulder pain: The shoulder is controlled by the rotator cuff, which consists of several muscles. If there is a tear in a muscle there will be pain with pushups. Even a cronic sprain or injury to the glenohumeral join labrum will cause pain. An MRI of the shoulder can help your doctor study this.

?: Not sure what you mean by locking up.  You may be simply fatiguing your muscles.  If you hit a limit, stop and rest and then resume.  You will be more effective doing less reps with breaks in between then powering thru till complete fatigue and max reps every time.  There is a role to doing max reps but try doing more intervals for awhile.

Depends: If it doesn't hurt or lead to difficulty or pain when moving the shoulder, it may simply be "normal." clicks can be caused by several different processes, and if there is pain involved, you should hold off on the push-ups for now and see a medical professional at your earliest convenience to make sure the bones, muscles, tendons and ligaments of the shoulder are all ok.
