One is observed: Generally speaking, muscle weakness is something a patient perceives and something that we can test for in physical examination by doing a manual muscle test, etc.  Atrophy is something we observe or visualize.  
In some cases they can be caused by the same thing (ex. nerve injury, immobilization, joint trauma), sometimes they are separate issues.

The muscle atrophy is even more concerning as this typically implies that there is nerve involvement with possible impingement that should be evaluated via MRI as soon as possible.
