my 6 year old son has enlarged lymph nodes for about 3 years now, one of which is 22 x 6.5 mm with no appreciable fatty hilum. ldh level is 248. he's an identical twin.  please advise.

Concerning: He should be seen by your family p[physician or pediatrician. A diagnosis must be made to make sure this lymphadenopathy (enlarged glands) is not cancerous. There are some cancers that are indolent and also blood tests must be done to rule out this possibility.

Lymphnode: Hilus is the area where the vessels run into the "bean shaped" lymph gland. The lymph gland typically has white blood cells in it, but as time goes on they diminish and are replaced with fat. Atleast no evidence for tumor which would be a proliferation of cells.

i have been being watched for lymphoma, and have multiple enlarged lymph nodes. the left lower side has been hurting, could that be my spleen?

It is possible: Depending on how low, could be your spleen, intestines, kidney or even ovary. Given your enlarged lymph nodes, an enlarged spleen is definitely a possibility. No: The risk of lymphatic cancer at that age is miniscule. Most are simple reactive lymph nodes that sift the body serum to remove debris. Vaccination. illnesses, throat infection, local bug bites or skin rashes often trigger transient enlargement. Occasionally a note becomes infected &amp; needs to be drained, but not commonly. Neck nodes ore often noticed since they are not buried in fat or tissue.

Monitoring Centers: The lymphatic system absorbs things too big to go into the arteries/veins like bacteria eaten by white cells and digested food. Lymph nodes "taste" the lymph to see if  there is bacteria/viruses/ cancers, etc. When it detects a problem, it alerts the immune system and your body reacts. They enlarge/hard in response to the inflammation created when they react.  Sx - pain, feeling the node.

It depends: It depends on the cause which can be either inflammatory (Infections like TB) or Granulomas like Sarcoid are not all that dangerous but if they are enlarged due to cancer, then it depends on which type of Cancer(Lymphomas are treatable but other Cancers are more serious.

Enlarged Lymph Nodes: I assume these were seen on a ct scan.  They could be from many different causes.  Lymph nodes enlarge from inflammation, infection, cancer.  I would talk to your doctor about the findings.  They know your history and can give you an idea about why they are enlarged.
