Visual obscurations: Your description of visual losses with heavy physical exertion is consistent with what happens in cases of what used to be referred to as pseudotumor cerebri now goes by the name of (idiopathic intracranial hypertension).  Hearing can also be affected if pressure inside the head suddenly goes up.  Rest typically relieves the condition.  You should visit a good neurologist for a diagnosis.

You Will certainly need a cardiac work up and if all clear from the cardiac perspective will need in depth medical evaluation.  Do not delay seeking care for these worrisome events .  Identifying a cause will allow treatment that may be life saving .

A few possibilities: If you're otherwise healthy with no major medical issues and you don't take any medication, you may actually be getting hypotensive (low BP). are you drinking enough fluids?  do you have heavy periods?  are you over-doing your exercise program?

my daughter screams intensely during the night, a few feet away from  me, will i get hearing damage from that?

: No, a child screaming should not cause you to have a hearing loss when a few feet away.You can certainly put earplugs in so that it lessens the intensity of the sound (for your comfort)-you will still be able to hear her. Certainly, you would want to figure out why she is screaming and help to resolve that.Not sure how old she is, but if it's because you're doing sleep training,it will get better.

i had from barotrauma last jan during a dive as i didn't equalize my ears effectively. ended with partial hearing loss and tinnitus. can i dive again?

Audiogram: The answer to the question is first, you must be told if it is a conductive or sensori-neural hearing loss.  The former may be surgically correctable and the latter may be possibly corrected with amplification.  You must have a hearing test (audiogram) and interpretation of the audiogram by an otologist.

Yes: Protect your hearing. Reduce noise levels if possible - watch the level you use a music player (like an ipod). Wear ear plugs or hearing protection when around prolonged loud noises - mowing the lawn, riding a motorcycle, etc.
