Depends: They are both complementary techniques.
MRA is more invasive and expensive and should NOT be the first image modality

Angio: If there is no obvious source for TIA other than carotids and with two conflicting studies of the carotids , then if it highly suspicious to be carotid in origin then you need to have conventional angiogram ( catheterization ) which is the gold standard or at least a third study with ct angiogram.

33 year old female with hypertension and received mra results that show mild to moderate vertebral artery stenosis. what does this mean usually?

Need to be watched: Means you have early narrowing of blood vessels in your neck. Which can progress and cause further narrowing and increase risk of stroke. No surgery needed now, but you have to be on high doses if statins and an Aspirin a day. Repeat carotid doppler every 2 years. If you have high blood pressure or diabetes , should be well controlled.

Possibly surgery.: For a young, asymptomatic female it may be the focal stenosis, (narrowing) is developmental, and no rx is needed. However, if there are  neurologic symptoms referralable to brain stem function, this finding may indicate need for surgical or neurointerventional treatment.

Doppler: Is technician dependent and mra can sometime overestimate the degree of stenosis. Cta is the test of choice for carotids. Angiogram remains the gold standard but not commonly done any more since it is more invasive and does carry a small risk of stroke.

NO: Those don't predict a stroke. You can occlude a vertebral (100%) and not have a stroke as the brain receives other blood supply that is connected. Be sure your blood pressure is controlled and keep it there!

i did an mra of the brain with no contrast and the results were normal brain mra without evidence of intracranial aneurysm, stenosis, or arteriovenous malformation. is the no contrast test good assurance or should i have done it with contrast ??

About decisions...: If after talking with his doctor, a patient decides to do an MRA without contrast, because the risks of the contrast outweighed the benefits of the contrast, then that must have been a good decision at the time. Humans can only make decisions that way, weigh the good and the bad, and then make the call. There is no other way (as we cannot see into the future nor travel back in time).

2017/ 2018/2019 all brain & cervical mra follow up on aneruysm all good. dec 21 mra now shows moderate stenosis in carotid arteries cervical spine how can this happen so fast? not a smoker 8st good diet excercise?

: You could ask a radiologist to review the prior studies to determine whether there was some carotid stenosis in those studies. It is felt by some experts that mild stenosis progresses slowly while stenosis of 50 % or more progresses more rapidly. I recommend consulting a vascular surgeon to determine how best to follow-up your carotid stenosis.

33 yr old female with hypertension and vertigo, blurry vision and chronic pain. mra of spine shows vertebral artery stenosis. could this mean surgery?

It's theLast resort : The first step is to determine if any of your symptoms are related to the stenosis.  The next question is the severity of the stenosis.  If you have no weakness, cramping, claudication (symptoms with exercise). This may not be the cause of your problems.  You should see a doctor with experience in this problem.
