My cervical mra results are: left vertebral artery demonstrates short segment focal high grade stenosis in mid portion. what does this mean? surgery?

Possibly surgery.: For a young, asymptomatic female it may be the focal stenosis, (narrowing) is developmental, and no rx is needed. However, if there are neurologic symptoms referralable to brain stem function, this finding may indicate need for surgical or neurointerventional treatment.

Depends...: Depends if it is a real finding or artifactual. Was gadolinium contrast used or not? Is it symptomatic? (why would you receive a neck mra at 33 yo?).
