Possibly: One should take bcp every day at the same time of day. If you take a pill more than 6-8 hours late, I recommend continued use of the bcp paired with the use of foam and/or condoms for two weeks to minimized the risk of pregnancy.  Another option is to use the morning after pill e.g. Plan B if you have intercourse during the 2 weeks after a missed pill.

Unlikely: If you've just ovulated , one does not ovulate again for approximately 4 weeks. If one uses an ovulation-day start of the pill, it may not prevent pregnancy on that cycle. Consider continuation of the daily dose of the pill.

Doubt it: A minute late?  Not likely to cause any decrease in the pill's effectiveness.

Yes: The pill is about 99% effective in preventing pregnancy when used precisely according to instructions, but it is not completely bulletproof.  All birth control methods (other than hysterectomy) have an associated small % risk of failure with subsequent conception.  So the bottom line is - yes - it is possible to ovulate while on the pill.  Go see your gynecologist; if pregnant, pill exposure is ok.

Clomiphene: The negative pregnancy test combined with the brown discharge could signal a pregnancy failure. Your doctor will want to advise you to probably keep trying, but a second failure, should it occur, is a sign to work you up for possible antibodies - such as anti-platelet antibodies - which can cause multiple pregnancy failures.

Withdrawal bleed: Stopping the pills is sort of like taking the 7 placebo pills at the end of the pack.  There's a sudden drop in the hormone levels and it can cause you to have an early period, though the period will probably be lighter than usual.  Understand that you are no longer protected, so will need to use a back-up contraception until you start your next pack.
