There : There is a popular acronym for determining warning signs of stroke: fast
f is for face: facial droop or crooked smile. A is for arms: inability to move arms or one arm being stronger than the other. S is for speech: can your loved one repeat a simple sentence or is their speech slurred? And t is for time: if you see any of thse signs call 911 as soon as possible.
If your wife is confused and not making any sense, this is of high concern and needs to be taken to the emergency room. If you suspect stroke, call 911 for timely treatment. Time is brain.

Atherosclerosis.: This usually means that she has hardening of arteries or atherosclerosis. Yes, she is at greater risk o stroke. However, risk can be decreased with proper attention to control of risk- factors such as blood pressure, lipids, smoking, obesity, diabetes and carotid stenosis. Emphasize to her the need to identify and control these risk factors with the guidance of a good doctor.

Most : Most often, severe, throbbing, pressurelike headaches, with nausea or vomiting, and with sensitivity to light or sound or noise are migraines.  Sometimes migraines follow a period of aura consisting of vision changes, numbness or weakness.  Sometimes the aura continues after the headache stops, but migraine aura should really be gone when the headache has gone away.  Migraine headaches can be diagnosed when there is a pattern of these sorts of headaches.

Some headaches feel like migraine, but they are symptoms of an underlying, serious problem.  Problems that can cause headaches that feel like migraine include subarachnoid hemorrhage, aneurysms, infections, and others.

New headaches, headaches that aren't like those that have been experienced in the past, headaches that go from 0 to terrible over a few seconds, and headaches associated with neurological symptoms such as numbness are all reasonable reasons to see a doctor promptly.

Rare: There is no point in being scared of things that are not likely to happen. Start living.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex. 
Get HPV vaccine.

: The words "chronic" &amp; "small vessel ischemia" imply poor blood flow leading to poor nutrition of the tissue leading to tissue changes. It does not suggest a stroke.

Call 911; chew aspir: A stroke is an emergency, can be halted or reversed if treated within the 1st minutes or hours.  The earlier treated, the higher the chance of a favorable outcome.  Call 911 to get rapid help, then chew and swallow a full-size aspirin.  Exceptions to taking an aspirin would be persons on aspirin, those taking coumadin (warfarin) or a NOAC (new oral anticoagulant), or those with very high blood pressure.

Not rare: Not sure if you mean lacunar strokes or tia's, but both do occur with high frequency , and if you experience, you should take meds to prevent such as clopidigrel or aggrenox.
