It is possible by history, examination, and blood tests alone an answer can be obtained, but often a kidney biopsy is needed to determine the cause. Treatment depends on which of the many different causes. Usually a nephrologist would work on this type of problem with you. Good luck.

Blood in Urine: Blood in urine is always abnormal and unexpected. Blood can be there due to infection, trauma, kidney stones, or cancer.  The cause of the problem must be identified.

See details: I doubt it is your bladder. If you have pain on defacation you need to be checked by a doctor. Anal fissures or fistulas are possible causes.

IC is not repeat UTI: Lower uti's are usually responsive to antibiotics, but recurrences are common.  Other conditions besides infection can cause frequency and urinary discomfort; interstitial cystitis is only one of many.  The hallmark of ic is visualization of 'glomerulation' on cystoscopy; however, ic may exists without seeing this. Potassium sensitivity test in the office can confirm.  You do need a diagnosis.
