i have middle back pain and it hurts to breath and  it is hard to get comfortable?

Doctor can evaluate: "hurts to breath" could be pain due to movement of the ribs if the movement irritates the rib joints and cartilage, could be due to irritation or pressure on nerves coming from the spine to that area, or due to other things like lung problems (infections, etc...), etc..., so if a doctor should talk with the patient and do an exam to get an idea of what might be happening.

A few possibilities: Your symptoms and their location could be consistent with problems in the back muscles (strain or fatigue), lung issues (infectious or congestion), and less likely but still possible urinary tract (infectious).  You've probably tried conservative things like heat, ice, Tylenol, (acetaminophen) and drinking fluids.  Now, it's time to make an appointment with your doctor before things get more complicated.

Pleurisy: Sounds like it could be pleurisy.  If you don't have a rash on your skin that looks like blisters (which would be shingles), this is likely an inflammation of the lining of your lungs.  Ibuprofen (if ok with your system) is the treatment of choice.  Other possibilities are a pulled ribcage muscle - if that's the case, Ibuprofen is still the treatment.

Difficult to say: There are many possibilities here. The pain could be coming from the spine. I am not sure how you know it is not muscular. Other possible pain generators include the lungs, ribs or the internal organs. This could be a pain that originates exactly where you feel it or could be referred from a more distant location. You need a good physical examination by an md/do who is used to dealing with this.

Cough and back pain: Symptoms of back pain, cough and pain with breathing suggests a respiratory or cardiac cause that could be potentially serious. The best approach is to see your healthcare provider and be evaluated for cardiac and respiratory disorders.  I suspect a chest x-ray, ecg, and lung function tests would be a fairly routine place to start.  Once identified, specific treatment can be given.

Costochondritis: Costochondritis is inflammation of cartilage between the ribs and is associated with a viral illness, smoking, and other causes such as trauma to the area, physical activity straining the rib cage, arthritis and other causes. Try ice on area, ibruprofen/tylenol to relieve pain, avoid activity that makes it hurt. Chk with doctor for source of pain. Chest xray/physical therapy may help.

Axial low back pain: If your pain is just in the low back then most likely this is just muscular in nature.  It should get better with time.  If not, then i recommend seeing a physician who can perform a physical exam and locate exactly where the pain is coming from.

Chest pain: Best to go to the doc and get checked out. Many possibilities and heart attack isn't high on the list for a 26y/o but not impossible. Pleural and pericardial inflammation are real as well as other things.
