Find the cause: Few pathologists these days set much by homocysteine unless it is extraordinarily elevated. If you have generalized stiffness with cramps when you're cold, and it's obviously much worse than most folks, a workup for myotonia congenita may be in order. Otherwise, a sports trainer might be the best person to visit first, to evaluate you and make some recommendations. Best wishes.

No: It does not. It doesn't cause premature arteriosclerosis either, but it once was believed that it does.

Doesn't drop glucose: It is extremely rare for metformin to lower blood sugar below normal since it doesn't increase insulin, it just makes your body more sensitive to it. Maybe your glucose was actually normal. Finger stick glucoses have a 10-20% error and usually read low. Besides, 500 mg of metformin is only half of the usual starting dose. Must be twice daily as only works for 12 hr.

More med: Depending on his a1c, he might need additional agents that will target pp bs. Metformin alone might not be enough to keep his bs under control, especially at the post prandial period.

Diet/Exercise/Metfor: Your aic is not in diabetic range which needs to be &gt;6.5 to be called diabetic
your aic of 6.1-6.2 make you in the pre diabetic range.Most physician will start you on a diabetic diet.Weight loss if overweight/obese, and exercise programme
some do recommend low dose of metformin.And metformin does not cause hypoglycemia
discuss your case with your family doctor or endocrinologist.

: Yes. A little b12 in a multivitamin is sufficient. It doesn’t matter when you take either pill during the day.

Diabetes: The high a1c suggests that your diabetes is not under control. You may well need insulin. See your doctor who may wish to refer you to a diabetes expert.

my free testosterone results: 6.6 on a scale of 0.1-6.4. little high. i have been trying to conceive with no luck. my doc. prescribed metformin. i dont have pre-diabetes. can the .2 difference cause this much issue? everything else is normal range?

: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, difference between 6.4 and 6,6 is not meaningful.    Wish you good health!
