Spine Pain Options: This pain in the distribution as you suggested is the result of an irritated nerve or facet joints or other injury typically in the lumbar spine (low back) which are caused by herniated disks, spinal stenosis or degenerative disc disease,  etc requiring further evaluation by a spine specialist and may be candidate for facet injections/radiofrequency ablation and epidural steroid injection.

Sciatica : Most patients with sciatica obtain relief with conservative therapy. If physical therapy with stretching exercises did not help you may need further testing with electromyography and neuroimaging, (lumbar x rays and mri). This will clarify the etiology of the sciatica. This could be due to a disc herniation, or spondylolisthesis (developmental vertebral condition). I wish you a prompt recovery.

Other strategies: Have you explored other means of helping with this pain like physical therapy, massage, therapeutic exercise, and activity modification. Also, simple interventions like trigger point injections might be valuable. You should try to abstain from a medicinal route as long as possible unless their use is only very intermittent. See a physical medicine or pain management doc to help with your concerns.

Physical therapy: I am not sure what you are asking.  If the answer below is not what your looking for please restate.  The most important long term therapy for the most common causes of sciatic nerve irritation is physical therapy.  NSAID's will help relieve the symptoms.  You can also try moist heat, cold, Tylenol (acetaminophen) and the over the counter sports creams.  You should follow up with your doc if it doesn't resolve

Further consultation: While no indivudal advice or diagnosis can be given here, "typical drugs" mught cover a lot or a little ground.  If one has chronic pain for years and having daily crying, it woudl be wise to get second opinions, maybe psychiatric consultation, or if antidepressants or pain managment is being done on its own, involving a clinical psychologist to boost depression treatment and behavioral pain mgt.

Pain: The sciatic nerve is a large nerve that is composed of many smaller nerves that come out of the lower back and run down each leg.  They merge and separate like an interstate highway system.  The nerve may be irritated by a spinal problem (herniated disc, spinal stenosis, arthritis), or a muscle spasm. In either case, sciatica is when the injured nerve causes pain going down the leg.

Sciatica cure?: Depends on the source of your pain. If the pain comes from a compressed nerve root in the lower back, a simple decompression or even an injection can do wonders! general stretching, strengthening and some anti-inflammatories hopefully keep you out of the operating room!
