Need more info: Medullary carcinoma is not organ specific and is a discription of how cancer looks like under microscope. Medullary cancer can be described in breast cancer, thyroid cancer for example. Thus the prognosis and treatment really depends on the origin of the cancer.

Even in the same organ, there's no one-size-fits-all remedy as no two people present with exactly the same extent of disease. A heads-up on medullary carcinoma of the thyroid is its tendency to run in families and with other endocrinopathies.

Not trivial!: Breast cancer, is most commonly of the invasive ductal type.  Often there are mixed patterns with medullary or lobular histologies.  Perhaps others as well.  The most important factors however aren't mentioned here.  Age, stage, grade, lymph node involvement all play a much more prognostic role at this point.

Resolution of U/S: Hi. Like all imaging modalities, there is a limit to what you can see with U/S looking at the thyroid. All cancers begin as a single cell gone bad that clonal expands (bear in mind possibility of multiple independent foci of MTC such as in MEN2's and FMTC), so at some point in their infancy, all MTC cancers are below the limit of resolution on U/S. Once nodules are biopsiable, FNA helps evaluate.

Theoretically yes: But unlikely.  Theoretically all you need is a tiny group of cancer cells at the microscopic level to have the capability to metastasize.  However in practice there is almost always a detectable nodule before it metastasizes because the cancer needs to be more advance and aggressive.

Biopsy diagnostic: Your biopsy has your correct diagnosis. Biopsy enables pathologist to look at exact tissues under microscope to describe your specific cancer diagnosis.  Radiologist is looking at imaging studies.  You DID have cancer before your thyroid was removed.  Please discuss your status with your surgeon and oncologist.  Best of luck!

Thyroid cancer: Hello. Unfortunately cancer of the thyroid will not show up on an ultrasound. A nodule or a growth in the thyroid can be seen and identified. A tissue or needle biopsy is needed to examine the cells for cancer. An excisional biopsy if needed will provide the pathologist the entire gland which then will be examined and a diagnosis rendered.
