Oxygen must pass through the bronchial tubes, air sacs and   interstitium to oxygenate the blood. In ild the  insterstitum becomes inflamed and scarred due to inhaled substances, drugs, arthritis, infection, cancer, or unknown  -- can lead to decreased oxygen level.
