see details: It is a test for lyme disease. A positive screen alone is not diagnostic. It needs to be confirmed by the western blot test which actually tests for specific antibodies related to the lyme organism.

Not very accurate!: Lyme testing leaves a lot to be desired. The screening AB via ELISA misses at least HALF of all cases. The Western Blot is a better test and should be done even if the AB test is negative, but most labs do inadequate tests; the best is by IgeneX but even that may miss over 20% of cases! : Diagnosing Lyme disease is quite complex. In North America, Lyme is uncommon this time of year. Positive IgM antibody for Borrelia burdorferi (the cause of Lyme) can help in diagnosis, but isn't the only test. Unless her doctors are highly experienced with Lyme, ask for consultation with an infectious diseases specialist. Good luck to her.

No: ANA (antinuclear antibody) is a relatively nonspecific test for the presence of a variety of autoimmune disorders. A positive ANA may indicate the presence of lupus, rheumatoid arthritis, sjorgren's syndrome, or dermatomyositis. ANA may be elevated in autoimmune thyroid, liver & bowel disease. It may even be seen with TB & hepatitis c. It has not yet been reported in association with lyme disease.

Nothing by itself: A single positive IgG band means nothing. Was the screen positive? How many other IgG and IgM bands were positive? You should discuss all of this with the doctor who ordered the test.

Yes,positive 20+ yrs: Research suggests igg antibodies can persist at least 20 years after infection. While igm antibodies appear first, unlike other infections they often persist ; are a good indicator of persistent infection. One may have chronic lyme with or without persistence of igg antibodies. Chronic lyme is difficult to fully eradicate ; no current test proves that it is no longer present in your body!

Yes: The presence of IgG usually indicates past infection which might have occurred months or even years ago.  The presence of IgM indicates a current or recent infection.

Complex: The diagnosis of lyme disease is usually on a clinical basis.  Serologic testing should not be done for screening purposes.  False positive results are very frequent in patients with autoimmune diseases, particularly rheumatoid arthritis, and particularly when these are IgM antibodies.

Probably not Lyme: Your Western Blot would need to be above 5 for a conclusive confirmation of Lyme disease. Unless you also had known tick bite, low-grade fever, "erythema migrants", you probably had a viral illness that should resolve on its own. Do discuss this with your primary medical provider or specialist if symptoms persist.
