Abdominal pain: It always worth seeing a doc to differentiate your signs and symptoms it could be some intermittent gastritis or maybe intercostal neuralgia so probably worth making an appointment with your pcp to determine the cause

Spasm: I am not sure of what all your symptoms are but most likely spasm. Of note this will happen more with stress or irritable bowel. It will also happen sometimes if there is days between bm or you are constipated.
See your doctor if possible. and drink much water to make sure you are well hydrated.

See doctor: Without knowing more of your history, it's hard to explain your symptoms, but some type of gastric irritation could be the cause. This would include things like gastritis, ulcers, irritable colon, etc. A thorough evaluation can probably offer you relief. Good luck!

Lump: Has a physician examined you to determine what the lump is?  Possibilities include inflamed gall bladder, hernia, intraabdominal or liver growth or rib pathology. I recommend  a consultation with an abdominal surgeon for a second opinion if you've already been seen by a physician.

Monitor: The usual cause of superficial lump is lypoma. Rarely could it be a small bony growth or neuroma.  A lypoma is usually painless, freely mobile, and remains stable over time. Bone spur is hard, fixed and tender on palpation. Neuroma may or may not painful but usually fixed.  Just monitor for changes in size, superficial ulceration, changes in color and if any suspicious change talk to PCP.

See your physician.: See your family doctor or internist, so that he/she can assess the "lump", and order any potential diagnostic studies to determine what it may be. It can be anything from an inflammed muscle, to some type of a mass.

Gallbladder: Right sided pain worse after eating a fatty meal could be gb.  Can cause nausea, vomiting and loss of appetite.  Can radiate to back.  I suggest seeing your primary for an eval and work up.

GERD: Sounds like gastro-esophageal reflux disease . Essentially the acidic gastric contents passing up into the distal esophagus due to an incompitent  esophageal  gastric junction. The distal esophagus has a different tissue lining than the stomach and can't tolerate the acidic reflux and this causes burning sensation.

pain like cramp or stitch in stomach on left side just under rib cage. 1wk ago started after going long trips in van. now occurs more. what's the cause?

Several causes: There could be several potential causes.  It could be causes by a muscle strain, costochondritis, spleen, colon, kidney, stomach, or pulmonary. If you are feeling better, it may be more of a strain.If symptoms persist, see a MD. As you where on a long van ride, I would worry about a pulmonary embolism.If your symptoms are associated with any shortness of breath, I would consult your physician ASAP

Surely: Surely you know we can't answer your question without an examination.  What you describe as a fatty lump may be something else entirely.  Please see your GP for a definitive answer.
