: You should see your gynecologist so that you can be examined as the first step to diagnosing your symptoms. Occasionally, there is a cycle where ovulation does not occur and a period is missed. A blood test can be done to determine if you are pregnant or not-the home pregnancy test may be incorrect. So, see your gyn doctor for diagnosis/treatment. Best of luck to you!

Wait another week: Choose a high quality pregnancy test such as CLEAR BLUE.  If still negative and you feel there's a potential False Negative going on then, you'll need to get a blood test.

Multiple causes: If you had unprotected sex, you may be pregnant or having a miscarriage. 
Otherwise, if you have not been sexually active or are on reliable contraceptives, you are experiencing changes associated with menses. Painful and /or irregular periods can be relieved with medications. See your gyn

Urinary tract infect: At 21 i would be most suspicious for a kidney infection.  I would also recommend a pregnancy rest.  If symptoms persist it would be a good idea to get checked.

Not listed at all: Neither fatigue or back pain is on the list of side effects. But, progestin excess (or deficit) can be caused by birth control pills in some women - it is not the same as a side effect it seems. Fatigue is a symptom of that excess. Check with the doctor who prescribed to see if you should try a different formula birth control pill.

Backpain: As you described pain with walking need to think about claudication which is from poor circulation, Need to be examined with some testing to get the diagnosis of vascular, neurologic or musculoskeletal origin. See your doctor for further work up

You're young: Irregular periods are very common in young women as their hormonal cycle is maturing. If other causes have been ruled out then this is probably the reason. If you are overweight, and have any issues like diabetes or vision problems, have your doctor check your prolactin levels as a pituitary tumor can be a rare, but treatable cause of irregular periods in young women.

If by "fatigue": you mean WEAKNESS you might have a pinched nerve in your back between the 4th and 5th LUMBAR vertebrae! Discuss with your Primary Care Provider!

This may be a : Presentation of a urinary tract infection  or variant of a systemic disease like diabetes.   Other possible causes could be entertained but just on the basis of your symptoms you should go to your physician to be evaluated.
