Surgery: Placental accretas are difficult surgeries.  Infant delivery is usually not the problem, hysterectomy is.  A well-qualified ob/gyn will ensure correct surgical personnel, ample blood products, clotting agents, ureteral stents, and embolization catheters are available for delivery.  Even in the best of hands, these surgeries require large blood transfusions and are considered high risk.

Here are my thoughts: Placental infarcts, if they occur in an ongoing pregnancy, would lead me to have concern for the well being of the fetus. So addressing the underlying disease that caused the infarcts is important. In addition testing of the fetus for its well being should occur. I might also treat the mom so that the fetus received the best placental function possible, examples: rest, hydration & possibly oxygen.

Strep milleri: This organism can be quite malignant, if in fact it is causing infection. It is not possible to tell whether its presence could have been detected earlier without more info on your symptoms and signs.  Hope that you and your baby are getting along well and will recover uneventfully from anything this may have caused.

Plac  covers cervix: The placenta can be located anywhere inside the uterus. If the placenta covers the opening of the cervix then it is called a placenta previa. In some cases it is not covering the opening but is close. This is referred to as a low lying placenta. This is common early in pregnancy and is monitored by ultrasound. If the placenta is covering the cervix then a C-section is needed for delivery.

Yes: It is okay to have intercourse during pregnancy.  In the setting of a low-lying placenta, it might also be okay if bleeding has never been an issue.  If you have been told to not have vaginal intercourse, it will be okay to use clitoral stimulation instead.   This should be checked with your ob/gyn as well, though.

Marginal previa: A marginal previa or a low lying placenta is usually diagnosed by ultrasound. Early in a pregnancy ultrasound can over diagnose the condition. Risk will depend upon whether you have had any bleeding with the diagnosis. A 28 week ultrasound is usually recommended to better visualize where the placenta actually is.

Avoid sex: for now, avoid sex. The low lying placenta usually resolves with time as the placenta migrates to the top. 
Wait till getting a clear from your OB prior to resuming sex.
