: You should be immunized. HIV infection or other immune deficiencies do not increase the chance of side effects or reduce the effectiveness of the COVID-19 vaccine (and probably not other vaccines either). On the other hand, you ARE at higher risk of serious infection if you catch COVID. You're a very high priority for immunization. Go for it!

A classic: Hi. A classic for that profile would be HIV. If you are at risk, I'm sure your doctor has thought of that. Why was the CD4/CD8 profile ordered? Have you had any opportunistic infections? If HIV negative, I suspect your doc may refer you to an infectious disease specialist. Good luck!

PRELEUKEMIC: it may be preleukemic. I would suggest a second opinion get copies of all labs and records. Also make sure a bone marrow biopsy and CTscan of chest/and pelvis have been done. If plasmacytes have been identified, a work up for multiple myeloma would also be indicated.

Complicated: There are many potential illlnesses which can affect the immune system, some of which are well known, but others which are as yet poorly defined.  You need to be seen by an expert on the immune system and be thoroughly evaluated.

No: Reference ranges are set so that a few percent of healthies fall outside on either side. Only if you're way out of range should these be a concern. Your personal physician knows when to work these up. I trust you know your HIV status; advancing HIV infection is the only illness I know that is likely to drive both into the way-low, and if you're HIV-negative and near-normal range I'd relax.

Response: Doubt that the meningitis permanently adversely affected your immune system.  The basic measures can vary slightly, and unless you have HIV causing this, would find little if any concern regarding a slightly low CD-4 count.  However, not knowing more particulars, can only comment generically.

How low?: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, it may have helped had you provided the actual test results, however, these findings are an unlikely cause of your symptoms.
