Irregular heartbeat: An irregular pulse is usually due to irregular heartbeat which in turn is often due to benign variation in electrical activity of the heart.  This is particularly true if not accompanied by other cardiac symptoms, progressive nature or exertional symptoms.  However, an ekg is appropriate and required if any other symptoms are present.

Loud heartbeat: Many times a "loud heartbeat" at bedtime may be indicative of hypertension.  Are you having incresed salt content in your diet near bedtime?  Sometimes alcohol consumption or foods high in sodium will increase your blood pressure at bedtime causing these symptoms.

Psycho tachycardia: If you have psychogenic tachycardia you likely are fixating heavily psychologically on your heart. It would be understandable that you'd feel what perhaps others wouldn't.

No distractions: It may be from the fact that it is quiet at night and that there is nothing else for the brain to focus on. It could be that her blood pressure is higher lying down than sitting up. The carotid artery sits close to the eardrum, so the force of blood through it may cause a sense of a "heard heartbeat".

Hard breathing: Blockage of nasal passages with harder breathing can make the heart work harder which means more forceful beat and faster heart rate with exertion. Are your nasal passages blocked because of allergies or because of infection? You may need a course of antibiotics if you have a sinus infection.

Stress management: Your symptoms are consistent with chronic anxiety and tension and if checkups reveal no pathology, I think you would be helped immeasurably by stress management therapy. I recommend that you get a referral from your physician  to a psychologist or psychiatrist(can prescribe medications if advisable).
