Try Lotrimin (clotrimazole) : Covering it up sometimes helps with the scratching.  Also I would recommend trimming and filing his nails and treat the ringworm so that it doesn't itch as much.  U can also give Benadryl (diphenhydramine) for the itching

Ringworm vrs other: Check this site  with pictures http://www.Pennmedicine.Org/encyclopedia/em_printarticle.Aspx?Gcid=001439&ptid=1.  If you have some certainty what u have is tinea (ringworm) you may try antifungals for athletes foot.( lamisl or lotrimin (clotrimazole) antifungal cream) it may take more than one month to really notice some  improvement. If you have too many lesions you may need pills to treat it. In doubt go to dr.

May not be ringworm: If over the counter ringworm remedies have not worked, it may not be ringworm. It is not feasible to provide a meaningful opinion without examining you and you should see a doctor.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex.

i've earlier used terbinafine ,clotrimazole and terbinaforce  cream... but ringworm is not decreasing... so please suggest me... how to cure this ringworm... i'm suffering a lot.... itchiness is there.. ?

May not be ringworm: The basic adage that if the treatment isn't working, the diagnosis may be wrong may apply in this case. The agent in lotrimin (clotrimazole) should  help if it is really ringworm, but if not better after a week you might want to have it looked at.Many forms of  dermatitis can have some similar appearance.

Sounds like ringworm: You are probably right.  It is likely ringworm.  
Hydrocortisone cream will only mask or prolong the problem.   You should obtain over the counter Clotrimazole or terbinafine cream.

These medications, creams and lotions, are usually in the "foot" section of a pharmacy or grocery store. You should be certain of what you are treating and if it does not improve with treatment see your pcp.
