Ask your PCP: Your pcp should be able to recommend a urologist that has expertise with vas reversals.  Make sure to ask what their success rates are with pregnancy rates, as opposed to patency rates.  Also, pregnancy rates are significantly decreased if the vasectomy has been longer than 8-10 years.  At that point, ivf with sperm recovery from the testicle (tesa) may be the better choice.

Specialist: A specialist in infectious diseases should be sought out. Lymes disease is sometimes very difficult to diagnose as it can mimic many other diseases. I would contact the ucla or usc schools of medicine.

Absolutely: There are terrific physicians in both geographic regions who specialize in diagnosis and treatment of back pain. If you have not been seen by your primary doctor that is a good place to start.  Alternatively, you can see a pain specialist like myself who specializes in diagnosis and treatment of spine pain or a spine surgeon for workup.  It might be jumping the gun to start with a surgeon.

Definitely call Dr: Many communities &amp; hospitals have special hotlines you can call to ask about COVID 19 testing, or you can call your Dr &amp; probably get an order for a test. Your travel history &amp; symptoms would warrant a test in my opinion. If it is negative a Dr evaluation to evaluate for other causes like flu would be advisable

You : You have enormous challenges you are facing and it's wonderful that you're allowing yourself to look for support and ask for help.  Ucla medical center has a wonderful department of psychiatry and their training clinic will give you good, low cost care.  Most clinics connected to academic hospitals base their fees on income. If ucla doesn't work out, i would suggest calling the other hospitals in your area and ask to be connected to the outpatient psychiatry clinic.  Finally, you can call 411 and ask the operator if los angeles has a hotline for mental health referrals.  Many cities do.  Stay focused on health and you'll get through this hard time.
