i recently found out i have a lobulated kidney with a exophytic componet ( left kidney) and a mildly enlarged spleen what could this mean?

Kidney OK but spleen: The kidney is OK but the spleen needs  clarification. Why is it enlarged. Ask your physician.  We need more data.

: A normal variant of kidney development. Assuming there were no cysts or stones etc no worries.  But your dr knows much more about you than I do.

I wouldn't say they: are common, but not especially unusual. You list three findings in the kidney...a bit unusual..and I would definitely pursue a kidney evaluation when your acute problem is resolved. But your current symptoms don't especially suggest a kidney cause. Continue to work with the ordering doctor, your primary or Urgent Care/ER provider, for diagnosis and management of your current symptoms. Good wishes

Explanation.: Lobular kidney and even  kidney stones could be the accidental findings on radiological exam and benign condition. Kidney related pain usually radiated to flank, more rare on back. Please check with your doctor.

Genetic: It could be genetic or that's what you were born with. If any other concern then should have a repeat sonogram in 6 months to rule out possibility of cysts and mass. Ct may be beneficial is sonogram is not conclusive.

Maybe: It's depends on the characteristics of the mass. Kidney lesions are very common and most of the time are no cause for worry.  Certain radiologic characteristics of the mass can signify possible cancer in which case surgery would be necessary.

More tests needed: It appears the next step would be to get a ct scan or MRI with contrast (if you can tolerate), as I am sure your physician or radiologist has already recommended/ordered. There are many possibilities and this is just a very scientific description of what is seen on the ultrasound, but the radiologist cannot tell you what it is yet. After the ct or mri, a biopsy or surgery might be necessary.
