DERMATOLOGIST: Hello.
There could be several reasons why your tattoo itches. It is difficult to determine what is causing it without a proper medical evaluation.
I recommend you to visit a dermatologist so he or her can check it out with a magnifier and try to find the reason why it itches.
Try using calamine or a anti-itching cream, because the more you scratch, the more it will itch.

Tattoo: It could be an infection starting but it could also be a reaction to the tattoo ink. Is the involved ink color red?

Depends: If these bumps itch, it may be due to a contact allergy developing to the dye (metal usually) pigment used in the tattoo.  In that case, the offending pigment will have to be removed surgically or by laser (which really hastens the removal ). In the meanwhile, topical hydrocortisone cream may help. If it does not itch, then it may from irritation related to the pigment which is often unknown.

Allergic to pigment: The pigments used in tattoos are not regulated and thus you may well become allergic to one of the elements (often a metal) in the tattoo. Although topical cortisone cream may help. the reaction will continue or even progress until the offending agent is eliminated.  Laser tattoo removal may hasten the elimination but it still takes time.

Not certain: But reportedly small red bumps may be due to overly sensitive skin, shaving skin prior to tattoo or mild allergic reaction to balm/lotion/ ointment that has been applied to skin to keep it moist.

People can actually: get allergic reaction, infections, warts and other reactions to tattoos. 6 months is way too long, get in to see dermatologist for correct diagnosis and treatment

See dermatologist: This can be an allergic reaction to the ink and or small granulomas forming in the skin around the ink.
They may be able to inject a steroid to calm it down

Mgt workup: It is possible that he has reacted to the ink, or that there is a developing infection.
I suggest applying Neosporin to the area, keeping it clean and dry. If the reaction worsens, a follow up is recommended.
