See a DR.: Each side of your scrotum should have one testicle. If you can palpate an additional mass its time for an in person medical examination. The extra mass can be a varicocle, a hernia or even a tumor. Do not delay getting an exam as these can be treated with appropriate diagnoses.

Pain in epidydimus?: You've had it for over a year so it must be tolerable. You can have a repeat U/S to see if it has grown in size. Go and discuss your situation with a really good pediatric urologist.

These are usually unaffected by size unless they are atrophic, shrunk following mumps orchitis or you have a syndrome. Basically there is nothing to be done for small testicles. Hormonal treatment, in the long run, can only shrink them further.

Left testis pain: While there are numerous possible causes of scrotal and testicular pain and discomfort, a common one, especially on the left side, would be a varicocele. These dilated varicose veins often begin aching late in the day when you have been on your feet for long periods and are improved by elevation of the feet. See your physician and be examined to determine the cause.

See below: Minor variations in size r usually normal. Marked difference in size should be checked out by urologist for varicocele or hydrocele. Also see doc if pain in testes or swollen, red, hot, painful.

Yes but not strictly: Small testicle size may be related to decreased volumes.  Certainly your husbands testicles seem small (<15 ml is small).  Given that his volume seems low ( less than 1.5 ml is considered low) he should probably have this evaluated to consider androgen deficiency.
