Emergency: Blockage in the left main coronary artery is considered an emergency condition requiring  urgent intervention by angioplasty or  bypass surgery. This situation is associated with high mortality if left alone.

Bypass surgery: everything is a balance between risk/benefit.  often with a &gt; 50% left main blockage, a bypass surgery is the safest choice.  medical therapy alone carries a high risk of sudden death due to the severity of the blockage.  a stent placement there, though relatively simple - might have a catastrophic outcome if any slight thing went wrong.  a stent is indicated if surgery is higher than usual risk.

Carotid: Having a significant carotid lesion increases the chance of having an episode related to it. Generally we treat with anti atherosclerotic meds and aspirin. Sometimes we do prophylactic carotid endarterectomy if the clinical situation supports that step.

Heart attack: It depends on how bad the blockage is. If severe it could cause actual damage, a heart attack. If less severe it could cause chest pain on exertion. If it is minor, there could be no symptoms at all. The symptoms can be chest pain, shortness of breath, or fatigue. As that artery supplies a large area of the heart muscle, it has been referred to as the widow-maker.

No: Circumflex is seldom a reason for single bypass or redo. Also seventy % is borderline for angioplasty or stent unless other methods show it is flow limiting (ultrasound or pressure wire)

Freeze Carotid: This is a very risky situation.  This procedure does not work and is very risky with 100% occlusion and even 85%.  If he smokes he should stop now!

Be evaluated: A 69% carotid artery blockage on one side isn't enough to cause dizziness/ fainting spells. Is it low blood pressure, anemia, a middle ear problem? It's best to have your internist work through the possible causes. With regard to your cerebral circulation, it would be helpful to know how your vertebral arteries look. A simple ultrasound can evaluate these arteries which supply the balance center.

i am going in for a blockage on my right kidney i am stage 4 renal failure w/ coranary artery disease and am going in for a surgical procedure remove a 60% blockage on my right kidney and 2 large stones on my left how serious is this procedure

Although : Although nothing is without risk, removing kidney stones is a fairly minor procedure these days. Most urologists will use lithotripsy to break up the stone, or do a cystoscopy (look up from the urethra into the bladder and ureter) and use some mechanical way of extracting the stone). By "60% blockage on my right kidney" i'm assuming you mean a kidney stone blocking the urine outflow. Because repairing a blocked blood vessel to your kidney is a somewhat more complicated surgery, and my previous advice does not apply to a vascular repair of your kidney's artery.

i am 54, previous heart attack.  now have mild blockage in the right carotid artery and moderate/ severe in the left so i am told.  cta shows 50% blockage on both sides.  the left carotid artery has a velocity in the 400's. how should they treat this?
