60 yr old male.  athletic and active.  radiologist report says severe diffuse grade iii and multifocal grade iv chondromalacia involving posterior weightbearing aspect of lateral femoral condyle.  knee replacement suggested. love a 2nd opinion?

Options: Those findings do not mean surgery only option. I would definitely try injections (multiple options), bracing and therapy first. Taking a daily arthritis medicine should also be tried .

MCL : Fortunately most mcl injuries can heal without surgery.  It does depend on where the tear is.  The bone bruise should resolve over time as well but it takes 6-8 weeks to heal this injury. If your knee feels unstable or keeps giving out,  then be sure to see your orthopedic surgeon.

Orthopedic Doctor: You may need an orthopedic specialist or sports medicine doctor to see and assess you.  Most likely surgery is not needed, but it will take time to heal. Let me explain: When you have grade 4 chodromalacia, it is a bad sign that joint is going to cause more problems. 
Several options 
1-abrasion and microfracture surgery.
2-laser-assisted treatments.
3-autologous matrix-induced chondrogenesis.
4-autologous chondrocyte implantation.

Yes according to : Mayo clinic, electrical stimulation. Electrical currents may encourage your body to grow new bone to replace the area damaged by avascular necrosis. Electrical stimulation can be used during surgery and applied directly to the damaged area. Or it can be administered through electrodes attached to your skin.

REST: Apply ice  try and rest,  it will take  a little time for the bruised bone to improve. Hematoma may have to be drained.

Follow directions: This injury can be unstable and proper immobilization will potentially avoid otherwise unnecessary surgery. My opinion is that your orthopedic surgeon will eventually put you into a hinged  brace to allow for range of motion without side to side movement.  Expect the first six weeks to be more strict guidelines and after six weeks guidelines will be lifted as healing dictates.

See PCP: Need and x-ray  for proper evaluation. See your pcp for referral to an orthopedic surgeon be fore treatment.

i sustained a hairline fracture to my lateral femoral condyle. doctor gave crutches and a hinged knee brace. can i use just my knee braces or are the crutches necessary as well?

: Truthfully, only the orthopedic surgeon that reviewed your x-rays can reliably answer this question.  On this forum, we would not be able to determine the risk of that hairline fracture advancing with weightbearing.  I suggest giving the doctor's office a call and getting an answer directly from that individual regarding the necessity to use the crutches.
