Monovision: Surprisingly, depth perception is very good after blended vision with either contact lenses or glasses.  The best way to confirm is to just try blended vision with contact lenses, so that you can be certain that your depth perception is adequate prior to undergoing lasik.

Reshaping the cornea: First a flap is created on your cornea with a blade or laser. It is then lifted and a laser is used to reshape your cornea (flatten it or make it more steep), then the flap is put back down. It's blurry for a few days and feels scratchy. Eventually, your distance vision should be excellent.

Several: Most lasik comes out well.  Some with a proclivity to eye dryness may have short term or sometimes chronic eye dryness afterwards.  There are complicated measurements and algorithms to ensure the power is correct but this does not always come out well due to underlying biology so sometimes the correction is not complete and may need further work (enhancement).
