hi i am 6 weeks 5 days. dr said gestational sac is large and irregular. last week hcg was 90, 000. hb144.  is this a bad sign for viability?

Possible: If there was no fetus and / or no yolk sac, then the pregnancy is not likely to be normal or viable.  By 6 weeks and five days there is always a fetus and a yolk sac and there is almost always fetal heart motion.

if my gestational sac is 13mm could i be 3 weeks pregnant and perhaps its just a big sac?

Possible: It is possible but that sac usually corresponds to a later pregnancy. What matters most is the us 1 week later to see what happens inside the sac.

my beta hcg came back at 788 at exactly 5 weeks today... my ultrasound showed a gestational sac at approximately 4mm big. is this normal?

Usually repeat: Too early. If you are 5 weeks then it is too early to see a fetus on sonogram. Typically the doctor would have you come back for a repeat sonogram in the next 1-2 weeks.

Heart beat ?: The most important thing to find out was if there was a fetus with a heartbeat visualized on the sonogram located inside of the uterus. If so then this is good news. The size of the gestational sac is not likely to be a contributory data point if the rest is true. You can ask your doctor to explain what was meant by a small gestational sac

Don't worry: Subchorionic hematoma (SCH) is the most common abnormality found on ultrasound.  SCH tend to be more common in the first trimester and is often associated with vaginal bleeding.  SCH do not appear increase the risk of miscarriage, regardless of size.

before i miscarried my ultrasound read: gest sac: 29 mm 8w 0d crl: 4 mm 6w 1d +/- 0w 4d could i have a complete molar pregnancy? or is only partial an option?  my gestational sac was measuring large and a crown rump length was measured.

Hard to answer: Ultrasound features – Partial moles are often indistinguishable from complete moles on ultrasound. However, demonstration of fetal parts favors the diagnosis of a partial mole. Histological analysis of the aborted tissue  is usually helpful in confirming the definite diagnosis. Your case demonstrates significantly enlarged sac which may happen with any molar pregnancy.

Probably: I would need to know more about the situation to know for sure.  In general its a good thing to see the gestational sac on ultrasound.  Its better if you also see a fetal pole and a heart-beat but these are seen a little later in pregnancy.   Best wishes!

Yes: The yolk sac typically appears at 5 1/2 weeks, and the embryo by 6 weeks, on transvaginal ultrasound, so 'empty' sac at 5 weeks can be normal.

I'd guess: Around 5 weeks as counted from the last menstrual period, but gestational sac size varies widely and is not a very good measure of gestational age.
