You can see: An orthopedist. Grinding in a joint is not a good sign and it sounds like you are compensating in your walking and causing other damage in other joints so have this looked at.

Absolutely: Patellofemoral syndrome is a maltracking of the knee cap when you bend and extend. Most of the time this occurs due to an imbalance across the knee with a weak quadriceps and tight hamstrings creating anterior knee pain and a grinding sensation that gets worse with squatting, kneeling, and stairs. Pt will help bring balance back to the knee and restore normal knee tracking.

Patellofemoral Syndr: Pf syndrome is the most common cause of non-traumatic knee pain (and clicking) and is especially common in young females.  It is caused by an imbalance between the knee hinge and the muscles of the thigh, the quadriceps, and often develops after a recent period of increased physical activity like sports.  Fortunately, it's easily and effectively treatable with physical therapy so go get some!

Osteoarthritis : I wonder if you are or were involved in a lot of contact sports in your youth.  Osteoarthritis or oa can cause the grinding symptoms that you are describing due to degeneration or breakdown of cartilage in the joint and loss of menisceal fluid that helps to lubricate the joints.  An x-ray would be helpful to get an idea of how your joint space looks and to look for osteophytes (bony overgrowths).

Chondromalacia: When the knee is flexed, as in going up and down stairs, a lot of pressure is placed on the joint between the undersurface of the knee cap and the femur.  If the cartilage in this joint is softened or arthritic, this can cause the creaking you describe and is also known as crepitance.  Thank you for the question.
