Not a concern: Pancreatic atrophy on ct is not a concern. The potential issue is pancreatic function which sounds like it is normal. Overall, i don't see anything to worry about.

ESR and CRP: Erythrocyte sedimentation rate, ESR or Sed rate and C-reactive protein are general markers of inflammation. If both are elevated, either one can be used for follow-up to assess the effects of treatment.

Here are some ...: Your feeling inflammation in right kidney needs to be objectively confirm with detailed Hx, physical exam, and urine test. In fact, most likely, what you have felt may be just muscle ache, and drinking a lot of water merely forces you to urinate more often. Flomax (tamsulosin) is to help relax and open the bladder neck and prostate urethra so you may see better urine flow but nothing to do with inflammation.

what is the significance of cm differentiation on kidney ultrasounds? e.g. is it a marker or inflammation or kidney function? just trying to improve my understanding?

: CM diferentiation = CorticoMedullary differentiation. By renal US, the cortical and medullary sections (outer and inner) have different echogenic charaters and that difference is showed as corticomedullary differentiation. If the cortical section (renal parenchyma) is damaged and scarring (chronic) the echo character changes so there is no more echo differentiation between the two sections.

Unknown: The ct changes could be from prior infection, obstruction or trauma, i would suggest a urinalysis and work-up to look for any signs of active disease and eval by your primary care physician.

Avoid acetaminophen: Try to avoid pain meds that are secreted by the kidneys. For inflammation Acetaminophen would not be effective, would recommend Ibuprofen as an initial treatment and see how you respond. If you have other concerns contact your nephrologist.
