Usually no: Most horseshoe kidneys r in fact 2 normal kidneys attached at the lower end. Some subjects have hydronephrosis due to obstruction at kidney ureteropelvic juction ; may require surgical correction. Very occasional patient might have severe persistent or recurrent abdominal pain which might benefit from division of isthmus joining both kidneys. Was fashionable procedure 50yrs ago, thankfully no more.

None: Shrinking kidneys are a sign of scar tissue developing in the kidneys.  There is no treatment that reverses the scar.  The are treatments to slow the loss of kidney function.  It is important to find out what is causing your kidney injury in the first place.  General treatments are avoid tobacco, low sodium diet, low protein diet, good BP control, good cholesterol control and avoid obesity.

Kidney damage: There are very few treatments that heal kidney damage.  However the kidneys have a capacity to heal with time.  Acute kidney injury can often heal by stopping medications that damage the kidney (nsaids, IV dye, etc) and making sure you are well hydrated.  If the damage is more long term (high blood pressure or diabetes), often you can only try to slow progression with BP control and sugar control.

Endoscopic endopyelotomy is the least invasive and requires several weeks of stenting&amp;#40; a tube left in the ureter to drain urine from kidney to bladder&amp;#41; until healing. Another option is laparoscopic or robotic repair but this can only be done by a urologist with extensive experience in this. Open surgery is, of course, the time tested treatment.

Preventive measures: For future kidney failure , shrinkage of size kidney , is a sign of  chronic renal disease , a progressive disease , effecting glomurali and nephrons , check your creatinine levels , control the blood sugar and blood pressure , two most common causes of kidney failure , eat plenty of vegi , fruits and exercises to prevent end stage renal failure.

No: Usually a malrotated kidney is an incidental finding not causing symptoms unless a different process is affecting same kidney or urether (urinary duct connecting the kidney with the bladder).
As such it does not need treatment for just being malrotated.
