Not enough info: Is he self- injecting illicit drugs? Are needles clean or dirty?  Does he routinely inject himself as part of a medical protocol or he is new to this? A baseball size knot is not normal regardless. recommend assessment.

Probably not kidney: Well, a mass on the kidney itself would generally be baseball-size before you could feel it through the skin.  You'd likely be peeing blood long before it got this big.  What you'r describing could be muscular, fatty, or a skin growth, but there's not nearly enough information here to speculate, and any unusual mass or growth especially with swelling should be looked-at by a doctor.

YES: Untreated acute cholecystis with impacted stone  at cystic  duct can rupture occasionally , will have severe pain, temp, signs of peritonitis tests will not be normal , WBC liver function tests will be abnormal .. Hope this what you are asking.

Just  a thought....: This sounds a lot like a SUBSAPULARIS strain/sprain!
These are small muscles that basically "attach" your "wingbone")scapular) to your chest and form an important part of your shoulder joint!!!!
Apply Heat..Rest that arm!!!!!\
and if still "in trouble" after 48 h contact your PCP

Epididymitis: Signs/sxs appear to be epididymitis of the testis, or orchitis.  You need a scrotal ultrasound and possibly antibiotics.  An incarcerated inguinal hernia, beyond pain and a knot, will eventually cause a bowel obstruction, which doesn't sound like your issue.
