No idea: If your doctor is a primary care physician he needs to send you to a Gynecologist.
If you are already seeing a Gynecologist please have a family or friends give you some ideas about other Gynecologists.

Use condom: It would be prudent to use a condom all the time, every time. Pregnancy is known to occur from sex on any and all days of the cycle.

See dermatologist.: If the steroid cream has not worked for months, you need to stop it.  Steroids have significant side effects and it makes no sense to take that risk for no benefit.  I suggest you see a dermatologist as soon as possible.

Thyroid: I doubt the IBS is involved. Menopause and thyroid disease are both possibilities though. See your doctor for testing.

Probably.: I believe that lack of sleep is probably causing the headaches , anxiety and the itchy skin. Hormonal changes  often brings changes in our body causing involutional melancholia. Please have your PCP check you up and get your hormonal levels and treat them if appropriate.

Vaginal pruritis: "itchy" skin around the vagina may be caused by yeast infections.  The "brown discharge" may be slow release of blood at the time of your period, which turns "red" to "brown".  See your gyn for an exam and testing, as there may be other elements causing this.

Not necessarily: An itchy vagina is not necessarily common during your period.  You may have an underlying yeast infection?  If still itchy, get gyn eval, best wishes.
