Granulocytes: It is part of CBC and would include neutrophils, basophils and eosinophils.
Other cells lymphocytes and monocytes are not granulocytes.

Multiple possibiliti: Some times environmental allergens could be there in your working place/home  , eg new linen, new deodorant, new chemicals exposure like soaps etc, seasonal allergies like exposure to pollens etc bringing out , Intestinal worm infestations - some of them can cause itching. Did you travel recently any where. Did you have any recent bug bits/mosquito bites. Dry skin is one of the common condition .

Trichophyton: The best medication is oral  Lamisil (terbinafine) for two to four weeks where there are large areas of fungus. See your doctor for a prescription.

Wait and see.: After a blood test, the hole will heal in a few days.  The itching is probably due to the alcohol used to clean your arm first, or may be due to the bandage tape.  Usually, when you stop bleeding, you can take the bandage off.  But if you are working in a dirty environment, then do leave it covered for a few days (unless the tape is making you itchy).

Nerve irritation: Possibly there was some nerve irritation that occurred during the blood draw. Give your arm some rest, elevate at night. Heat/cold compresses can be tried to stimulate your nerve. Hopefully, it will be over with in a few days.

Follow it up: These are borderline-high liver enzymes. It may be nothing more serious than your being sensitive to Ibuprofen or another medicine you took, only you know if you drink a lot, and  we need to rule out metabolic syndrome, a chronic hepatitis, hemochromatosis, wilson's, and a couple of others.

Tiny cut: If you are sexually active it could be a tear from insertion of the penis into your vagina. Try using a generous amount of lubrication to ease this process.

Your dermatologist seems to have ruled out common causes if itching without an associated rash. To be sure, schedule a follow-up evaluation with your dermatologist. Be specific: WHERE did you experience burning, itching? Exactly. How long did it last? "Lesions"???? Where? Exactly. What kind of lesions? Exactly. Why herpes test? How are you now? What was the diagnosis? Treatment? If you had first attack of herpes it would be painful, not itchy, and the blood test would not become positive until a couple of weeks later.
