: If you quit now, completely, in about 10 years you will be at your background risk.
You can reduce your risk of cancer by adopting healthy habits: Eat a mostly vegetarian diet, avoid alcohol, tobacco and street drugs, avoid excessive exposure  to sun, get HPV vaccine, exercise 30 minutes each day, drink enough water so your urine is mostly colorless.

No: Radiation itself is not painful but can have side effects such as tiredness, skin irritation, cough, shortness of breath and low blood counts.

Low on diff dx: In medicine, we often think of many possibilities for specific symptoms, called a "differential diagnosis list" and lung cancer for shoulder aches would be way down on the list, if even on the list at all.  I would see a dr and get the shoulder evaluated, properly diagnosed and then the best treatment would occur.  Good luck.

Seek Palliative care: A good pain specialist can control your pain. So you must seek medical advice from a palliative care specialist. Alternatively ask for help from a Pain specialist(they are available in every hospital whereas Pall. care expertise is not always available in smaller hospitals.

Assess options: If symptomatic, consider radiation to the areas of disease causing the symptoms. Chemotherapy can be considered, if agents available. Surgery can be done for palliation but typically is not used. A lot depends on how well your father is doing now with these treatments, as well as the extent of the cancer in the body. I am sorry to hear that your father has to deal with this cancer.

i have swelling in the upper inside right cheek.  it's not painful, unless i press on it.  
i have small cell lung cancer, was in remission for 6 mo.?

Needs to be checked: With your history of small cell cancer in remission, there are several possibilities, which may or may not be related to your cancer.
