Muscle damage: When heart muscle is injured troponin leaks into the blood where it can be measured. Elevated levels typically indicate some damage to the heart. There can be false positive readings especially in patients with renal failure.

Chest pain: Chest pain has many causes such as cardiac, pulmonary, gastrointestinal, musculo-skeletal.  At age 21 heart attack is unlikely.  Go see your doc and get it checked out. You will feel better when you understand the problem.

Back aches: What you describe is highly unlikely to be related to the heart. It sounds more musculoskeletal. If it is persistent should see your doc and be evaluated.

Heart Attack: Costochondritis is inflammation of the bone to cartilage of the sternum (breastbone).  The pain is worse when you push on this area.   A heart attack on the other hand is from the heart muscle not getting enough blood supply (and oxygenated blood).  Heart attack pain is not made worse by pressing on the sternum.  It often feels like a huge pressure on the chest, and may radiate into the jaw or arm

Costochondritis: Costochondritis is inflammation along the breast bone where the cartilage attaches to bone.   Does it hurt to press on your sternum?    If so, that suggests Costochondritis.    Heart attack symptoms often involve a pressure feeling on the chest, nausea, shortness of breath, pain in the haw or the arm.    When in doubt, seek medical attention immediately.

Yes but not likley: Chest pain can be from heart but in your case this would be less likely given age. Other possibilities would be pulmonary embolus (blood clot in lung), this is usually with shortness of breath, but not always. Also esophogeal reflux/spasm. Also, musculoskeletal causes are the most common. If it gets worse, or doesn't go away, or is with shortness of breath, seek immediate help.

Not usually....: A heart attack occurs when there is a decrease in the blood flow to the heart itself. Symptoms of a heart attack include chest pain or pressure that can radiate to the jaw, shortness of breath, sweating, etc. Spasms of the chest wall muscles can happen for many reasons, including after a lot of coughing, and is not usually a sign of a heart attack. If you have chest pain, see your doctor or er!

Can't say: The symptoms of heart attack can vary quite a bit.  Some patients have no symptoms at all.  Pins and needles in the shoulder blades are not the usually complaints before or during a heart attack.  If you are worried though you should get it checked out.
