Good question: This may or not be a cancer, but without changes in appearance in 2 1/2 yrs  and not knowing your complete medical history I would suggest that you follow your physician's plan and to feel better emotionally ask him/her to refer you to an oncologist for a second opinion. It is not unusual for doctors to agree to this request.

Terminology: A tumor that is confined to the ducts of the organ and is making mucus. Such neoplasms are not cancers in the traditional sense, but may exist with cancer and may be precancerous.

Unlikely: Colon and appendix are located quite deeply in your abdomen. So you can not feel any growth arising from there unless it become very large and it has been neglected and allowed to grow unchecked for many years.

No,: that would be very, very unlikely.

Operation: If there is a low suspicion for cancer, and other parameters are within normal limits, these lesions can be observed with repeat ct scans or endoscopic ultrasound examinations. However if there is a concern that the ipmn has cancer within it, then these should absolutely be surgically removed unless the patient is not healthy enough to withstand this type of operation.

i recently had a mri. the reported mentioned this: approximately 5 mm cystic lesion in the tail is likely a sidebranch ipmn without suspicious imaging features. recommends one year follow up mri.

should i be concerned?

No worries : Concern yes, but worried no. Follow up as prescribed with a repeat in a year. The words “without suspicious imaging features” are key to your findings.

Leave it alone: Most of these little things in the pancreas are harmless. Just watch it. If it starts growing, it can be removed while it's still not a huge danger. Pancreatic surgery is grisly. Avoid it if you can.

IPMN: IPMN's are growths in pancreatic ducts, yet you were told your lesion was a cyst and not a solid lesion or growth. Main branch neoplasms have more propensity to become malignant than side branch ones, but If I had your finding, I would want to be evaluated by a pancreatic surgeon or specialist at a university hospital for best care, advice and follow up. Surgery may not be recommended if no pain.
