how do i tell if the pain coming from my butt is sciatica or an internal thrombosed (blood clot) hemorrhoid?

Easy: Hemorrhoidal pain is local at your anus usually, sciatica usually radiates down your thigh to your calf/shin. Also thrombosed internal hemorrhoids are very unusual and otherwise internal hemorrhoids rarely cause pain. External thrombose often and do cause pain. You should be able to tell the difference.

Sciatica: Sometimes it is difficult to determine if the pain is coming from the lumbar spine, sacroiliac joint, or the hip. An MRI of the lumbar spine, pelvis, and hips and an examination by a spine or hip specialist will tell you where the pain originates. Anti-inflammatory medications, muscle relaxers, neuropathic pain medicine, and physical therapy can help to eliminate the pain

Tarlov cysts: NO. There is absolutely NO evidence that suggests that Tarlov cysts cause any symptoms

Not exactly: Classically siatica goes from buttocks to leg down the back this may be nerve compression but a different nerve root from your description sounds like L3  or L4  or other peripheral nerves femoral obturator, siatica feels like this but in different part of leg classically

Need an exam to tell: Sounds like it has components of muscular, joint, and nerve compression and unlikely to be as simple as a one word description.  You are a little young for degenerative spine conditions and Radiation of sciatica to the inner thigh is rare.  With your history of type1 diabetes you should see someone for an exam if the pain continues.

Give it a little tim: Sciatica, defined as pain radiating down your leg to the foot, is usually caused by a irritated nerve in your lower spine. It is not uncommon for symptoms to occur and later resolve. Anti-inflammatories and stretching can often help. If your symptoms are severe, have been going on for 6 weeks or more, or associated with weakness, you should see your physician for evaluation.

Symptoms of sciatica: "sciatica" is pain, numbness, and/or weakness in the distribution of the sciatic nerve. It's almost never caused by irritation of the sciatic nerve, but rather by irritation of the lumbar nerves from the spine that make up the sciatic nerve, ie. L5 or s1. The path of symptoms is from the buttock down the back and side of the leg, ending in the foot.

Sciatica: Depends on the cause. If it is due to a disk problem epidural steroid injections are worth trying - if no improvement surgery is always an option. If it is due to sacroiliac joint inflamation then injecting the joint will relieve it in 60 % of the cases. Occasionally I have to perform radiofrequency ablation for longer term relief.
