Intact hymen : An intact hymen is used in any cultures to signify virginity.  This not necessarily true.  Upto 19% of the sexually active postpubertal females  a intact hymen.  Clitoral stimulation has no effect on the hymen.

Normal Finding: The hymen covers the vaginal orifice in younger  women.  In some it remains completely intact, but it become "patent" in others.  Partially intact means just that, that it has partially opened.  This can occur naturally as well as with trauma.  There are no cuts or indicators.  It is seen on physical exam.

False: Unless a woman has an imperforate hymen, (exceedingly rare) there is no exam that is going to tell a doctor whether a woman has had sex or not. This is a false diagnosis if it's given. I realize there's widespread practice in which drs determine if woman has had sex, but virginal versus non-virginal hymens often look identical. It's a cultural/religious diagnosis that has minimal physical findings.

Anatomically: They are barely close to each other. What you do to your clitoris will not affect your hymen. All the best.

Intact Hymen: It usually would not be completely intact - because an imperforate hymen is a congenital disorder which would not allow blood to flow from the uterus out of the body. However, what is likely meant is that there is no tearing or other findings related to tearing of the hymen. You may or may not bleed with sexual intercourse. Clitoral stimulation should not have an impact.

Hymen: If it's completely intact,  there really might not be a way for blood out.   Sometimes it can sit within the vagina, which may lead to blood clots and infection.   This is definitely one to discuss with your gynecologist.

Find a therapist: By your comments your obsession with your hymen (or virginity) is causing an alteration in your ability to enjoy life. It is time to stop going to gynecologists unless you find one that is also a therapist. You need to find out what feeling or fear your obsession  is hiding. No: An ultrasound can not show such a thing. If the hymen is intact this can be seen during a physical exam. The problem is many virgins do not have an intact hymen. Physical activity can open it. If a doctor wanted to know if a patient was a virgin he or she would simply ask them.

It shouldn't: No, yeast infections should not cause damage to the hymen, even if it gets swollen.
