Time course: Hi. NPH &amp; R differ by their time course of action. While they are both now mostly relegated to the history books, some people still use them. R peaks sooner and lasts a shorter time than NPH. Engineered insulins (e.g., Lantus (insulin glargine) vs. Humalog or Novolog) also differ in the time course of action. This gives rise to the notion of "basal" insulin and "meal" insulin. Different insulins fulfill diff. needs.

Should be . . .: Novolin (insulin) n and Humulin N are just two different brands of Insulin nph w/onset in 1-2hrs, peak in 4-14hrs & duration of 10->24hrs. Therefore, your body might notice some differences even though theoretically they're the same. Ask your family doc or endocrinologist before switching. S/he may want to change the frequency of your self-checks. Ps if you're ever unsure, ask your doctor!

Age 54: At your age you most likely have type 2 diabetes. Unlike type 1 diabetes, type 2 can be controlled to a point where you won't have diabetes. Won't happen overnight but you must see your primary doctor and nurse practitioner to help you get rid of diabetes.

Different insulins: Long acting insulin=nph/levemir/glargine. These help control sugars when one is not eating since the liver keeps making sugar in between meals.
Ultra short acting=rapid analogs=glulisine/lispro/aspart=help control sugars that rise after meals. They are the closest we have in timing profile to natural Insulin action in non-diabetics.

NPH Drug Treatment: No definitive evidence exists that medication can successfully treat nph. Parkinson drugs have been reported to be of benefit in anecdotal reports, these patients with nph may represent misdiagnosed cases of parkinsonism.  In patients who are poor candidates for shunt surgery, repeated lumbar punctures in combination with diuretics may be considered.

i am type 2 dm on oral medication. now i am preganant. since i am on tablets instead of insulin can i continue my preganancy?

Yes: Some of the oral meds actually decrease the risk of miscarriage. Others do carry some risk and will need to be changed. There is also a chance that you will need to be put on insulin at some point in the pregnancy but you can with close monitoring have a healthy pregnancy and a healthy baby. Best of wishes.

U 500 insulin. : You may have a problem due to insulin antibodies?  Discuss with your team.  Check your glucose meter.
