Many possibilities..: This could be many things and i would recommend seeking complete medical evaluation to get an accurate diagnosis. Common causes of these symptoms can be shin splints, medial tibial stress syndrome, periostitis, stress reaction or stress fracture, or exercise-induced compartment syndrome.

Possibly: I have treated two patients in which the ultrasound was negative for clots but on repeat ultrasound 2 weeks later the test did show clots. However, in that area a possibility of a muscle/tendon sprain or tear is possible also.

Best to See a Doctor: This could just be a flare up of your varicose veins, it could be a superficial blood clot in the leg.  U said U have leg swelling, it matters if it is a lot or a little. Do U have any risk factors for blood clots? Smoking, Birth control pills &amp; inactivity all are risk factors 4 blood clots in the leg. Some clots are a nuisance, &amp; some a serious problems. If you fee a lump it is probably not deep

See below: It could be arthritis or injury.  We would need detail such whether or not your knee was swollen which would help us better determine what is going on.

Swollen leg?: Is that leg swollen?  Are you on any hormone therapy such as birth control or have you traveled by plane recently? Recent surgery or family history of PE or DVT? The pain is a somewhat vague symptom but if it's coupled with a risk factor and swelling then you should get checked for a DVT.

Unstable joint: Get an MRI of the knee and an orthopedic evaluation. The calf pain should be evaluated with both a venous and arterial doppler study of the lower leg. If the knee is giving out, i would suspect some internal derrangement.

Yes.: Calf pain and swelling may be an indication of a blood clot within the veins of the calf muscles, which we call deep vein thrombosis (dvt). This can occasionally travel in the bloodstream and enter the lungs (pulmonary embolus); this may cause sudden death. Most people with calf pain do not have blood clots, but in the right clinical situation, this needs to be considered and aggressively treated.

Not enough: For a meaningful answer, but among the most common causes and in the right circumstances musculoskeletal pain and clots or DVT can cause this symptom. Have a doc examine your leg if persistent pain.
