See  dermatologist: There are several causes for ingrown toenails from improper shoes to skin infections.  Sometimes these nails need surgical narrowing.

Removal...: ...Is the only permanent solution. Just the painful border can be removed, or the entire nail. After removal you can let the border grow out again, or your doctor can use a chemical to stop the nail from regrowing. If you're concerned, don't perform bathroom surgery on yourself. That can lead to an infected ingrown toenail, which could require antibiotics. And heels are never a good idea!

IF YOU ARE DIABETIC: doubt the infection in the ingrown toenail will cause systemic infection,or sepsis, or septicemia. 
unless you are diabetic or if you have a severe peripheral vascular disease, and you end up with a bad case of ischemia or gangrene.
either way why you want to wait until it becomes a life threatening infection, start soaking your foot, trim your nail well, see a doctor,take antibiotic,etc...

Not that common: By not healing well what do you mean exactly? I highly doubt you have gangrene, but may be an infection that is still there. Also there could be piece of the nail left behind that is causing it not heal well. Gangrene usually happens in people who have some sort of underlying illness with poor circulation. Please follow up with your doctor or a podiatrist for further evaluation.

It may hurt, but...: There is a small piece of nail still embedded in the corner. You need to get that out. If you can't get it out on your own, a podiatrist can. Sometimes this can be done without numbing the toe, but often, anesthesia is necessary. If it is a chronic condition, there is a very successful surgery that can be done to permanently correct the problem.
